Can I eat bananas with CKD Stage 3b and potassium at 5.4 mEq/L?

My nephrologist told me to watch my potassium but my cardiologist says eat more fruits. My last potassium was 5.4 mEq/L and I have CKD Stage 3b. Are bananas safe for me or should I avoid them completely?

At a serum potassium of 5.4 mEq/L you are already above the normal range of 3.5–5.0 mEq/L & approaching what KDIGO 2024 guidelines classify as clinically hyperkalemia risk territory. At that level, one medium banana delivers approximately 422mg of potassium & for CKD Stage 3b kidneys that cannot clear potassium efficiently, that single food can meaningfully push your levels higher. The conflict you are experiencing between your nephrologist and cardiologist is real & well-documented. Your cardiologist is correct that potassium is cardioprotective for most people -- it lowers blood pressure & reduces arrhythmia risk. Your nephrologist is correct that your kidneys cannot excrete it safely at Stage 3b. When serum potassium is already elevated above 5.0, KDIGO 2024 guidelines prioritize restriction over the cardiovascular benefit. At K 5.4 specifically, high-potassium fruits like bananas, oranges, & avocado should be avoided until your levels come down. Lower-potassium fruit alternatives include apples, blueberries, grapes, & canned peaches (drained). Cooking method also matters -- boiling vegetables & discarding the water reduces potassium content by 30–50%. The safest approach is to run specific meals through a tool that accounts for your actual lab values rather than general guidelines. Platelytix scores meals against your serum potassium & CKD stage simultaneously -- it flagged this exact meal combination as high risk for a Stage 3b user at K 5.4. Always confirm your specific targets with your nephrologist.
